If you made a Meeting & Event Reservation by separate form for the 2012 ASI International Convention in Cincinnati, Ohio, please use this form to secure your reservation, order audio-visual equipment or meal items, or to request a specific setup for your meeting or event.

If you have not yet made a Meeting & Event Reservation, use this form to do so, as well as to order audio-visual equipment or meal items, or to request a specific setup for your meeting or event.

You can make audio-visual and room reservation changes up to 2 weeks prior to the convention. You can make changes in meal guarantees up to 7 days prior.

You will receive a confirmation e-mail once your completed form is received. If you do not receive a confirmation within 7 days, please email Sharon Blumenberg at sharon.blumenberg@nad.adventist.org.

Menus for both hotels will be posted next month. Rooms will be assigned 30 days prior to your event if your completed form is submitted by the deadline.

If you need to change your order, please resubmit this form in full.

  • Special menu requests must be determined in cooperation with the ASI Event Coordinator.
  • The guaranteed number of meals will be the charged amount, even if the number actually attending is lower than the guaranteed number.
  • If your expected attendance increases, please notify the ASI Event Coordinator as soon as possible.
  • The guaranteed number of meals may be increased at the convention, but may not be decreased after the commitment deadline.
  • The reserving organization will be billed for food service and equipment charges following the convention.
  • ASI cannot guarantee equipment not requested in advance.
